Oct 7 was Israel's worst intelligence failure; the security chiefs who were in the chair took responsibility and many resigned — the prime minister has resisted a full independent state commission of inquiry into his own conduct. That's why each ignored-warning report matters: the accountability the rest of the system accepted, the top of it has dodged.
